(Seattle, WA) -- An investigator with the National Transportation Safety Board says the Ride the Ducks vehicle involved in Thursday's deadly crash with a charter bus had not received a recommended safety fix. Investigator Earl Weener says the fix had been recommended in 2013 by the Atlanta company that refurbishes the World War Two-era amphibious vehicles. The top executive with the Seattle Ride the Ducks company says the business is trying to determine whether it received notification of the recommendation when it was issued. The death toll from the crash has risen to five, with a 20-year-old woman dying yesterday at a Seattle hospital.
